Tidepool Animal Exploration Pre-visit Form
What is your school/group name?
When will Aquarium on Wheels be at your site?
What time would you like to start your program?
What is your name?
What is your email?
What is your phone number?
Do you have a cell phone number?
What age/grades will we be serving?
Although we do not limit the number of guests when you book a tidepool display, logistically we cannot serve any more than 80 - 100 guests per hour. At schools we typically serve 20 - 25 students every 15 minutes.
The touch tanks are located inside the truck. The vehicle must be parked on a hard, flat and LEVEL surface that is asphalt or concrete. The vehicle cannot drive over or park on grass, dirt or gravel due to the weight of the truck (12 tons!). We need a 50 ft x 25 ft space with a 13 ft vertical clearance.
If you have a set number of groups/people coming through the tidepool animal exploration you may find it helpful to create a schedule that designates specific times for each group. This is recommended if we are at a school during school hours. If your event is a festival, fair or birthday party a schedule is not necessary.

(we accept only the following file types (10MB max size) and they MUST include a file extension (ex: filename.jpg): .jpg, .png, .gif, .pdf, .doc and .docx)
